Parents please bring the following information on July 13th:
*Your Identification (Driver license, Social Security Card, Student ID).
*Pregnant Women, Postpartum Women up to 6 months after delivery, infants,
and children up 5 years old please attend appointment.
*WV Medicaid card or WV CHIP Gold or 30 Days of check stubs for the
Household.
*Social Security Numbers & Immunization Records if available
**If pregnant: Proof of pregnancy or Ultrasound
Families signing up for WIC will receive a free cookbook for completing the appointment.
