You are being asked to complete a brief survey because you have been identified as an important stakeholder in designing and/or implementing a collaborative system to address mental health in our schools.  The Expanded School Mental Health (ESMH) State Steering Team seeks your help in determining how we can better assist schools and communities in supporting your needs.

Because we are strategically involving as many stakeholders as possible, you may receive this email more than once.  If that is the case, please ignore duplicate emails and complete the survey only once.  Please feel free to forward this email to other relevant colleagues that you feel believe are important contributors to the survey.

Information gained from this survey will guide our work plan development when our ESMH team meets for strategic planning in late January.  Since this survey is time sensitive, completion in a timely manner is appreciated.  The survey will close at midnight on January 25th
